DESCRIPTION OF FIELD TRIP:

The Girls Choral Academy is excited to announce a special field trip on April 8th for the CONCERT CHOIR GIRLS, 2024. The choir will be performing at a convention being held at the Amway Grand Plaza Hotel for a convention. Your child will miss the first half of their school day. GCA staff will be dropping your child off at their designated school after the performance.

TRANSPORTATION CONSENT- I hereby give my consent for my daughter to carpool and ride in the vehicle of an adult Girls Choral Academy staff member. GCA will provide transport to and from this event, delivering each girl to their respective school after the peformance.

MEDICAL EMERGENCY CONSENT - I consent and authorize staff or volunteers from the GIRLS CHORAL ACADEMY to seek and provide emergency medical attention for my child in the unlikely event it were necessary on this field trip.
